Funding & Eligibility

Supportive Living services may be available through:

  • Community Access for Disability Inclusion (CADI) Waiver
  • Brain Injury (BI) Waiver
  • Elderly Waiver (EW)
  • Other approved funding sources

Eligibility is determined through an assessment completed by the county, tribe, managed care organization, or lead agency. Individuals must meet program requirements and be approved for Customized Living or 24-Hour Customized Living services.
